Photo/video release

I give permission to Sarah’s Violin Studio to use photos or videos taken during my (or my child’s) lesson. I understand that the first name may be shared alongside the media. These photos and videos may be published on the studio’s Facebook and Instagram pages (public pages)

• If the student is a minor: I am the parent/guardian and give permission for my child.

• If the student is an adult: I give permission for myself.
